I am a new client. What do I need to prepare?

If you are a new client, please download and complete all pages of the New Client Packet prior to your first appointment. Please email topsmartnutrition@gmail.com your completed packet at least 48 hours before your scheduled appointment.

If you or your child referred to RD for the poor weight gain, please complete 3 days food record (link) prior to the first appointment. Please use The Portion Guide (link) sheet to describe the amount consumed.

What can I expect at my first appointment?

Your first appointment will involve a pertinent medical and diet history, present diet and lifestyle assessment. In most pediatric cases we will need also to collect a medical and diet history from birth. We will set initial goals and a realistic plan of action. Please bring all copies of your most recent laboratory tests, medical problems, and a list of medications, herbs, and/or dietary supplements you are taking. For kids, please also prepare birth weight (if possible), and any historical weights or growth charts you have. Tip: for younger kids the yellow vaccination card may help to learn of weight trends.

Do you take insurance?

We are not currently a provider of any insurance plans in NY. Please check back for updates.
We are a preferred provider for Medicare, Horizon, BCBS, Oxford and United Health Care in NJ.
Please call your individual carrier prior to the first appointment and ask the following questions:

  1. Does my plan cover nutrition counseling for the specific condition I am coming in for (weight loss, acid reflux, diabetes, etc)?
  2. Do I need a referral from my doctor?
  3. Is there a limit on the number of visits?
  4. Do I need to pay deductible first?

You may also call our NJ office at (201) 791-9340 and we will gladly check the insurance coverage for you.
For all other insurance, we may offer you a coded superbill that can be submitted to your insurance company for reimbursement. You may call your insurance company ahead of time to find out how much they will reimburse.
Additionally, if you have a flexible spending account or health savings account, most plans will allow you to derive tax-free benefits with nutritional services.

Will Medicare pay for my visit?

Medicare Part B, will cover Medical Nutrition Therapy (MNT) for beneficiaries diagnosed with diabetes or renal disease (except for those receiving dialysis). Medicare will pay for 3 hours of MNT during your first year of diagnosis and 2 hours each subsequent year.

When my payment is due?

For the office consultation the payment is due by the appointment time.
For the online / phone consultation the payment is due 48 hours prior the appointment.
